Health-plan denials: internal appeal before external-review screening
Qualifications and requirements: A health carrier's denial letter or explanation of benefits identifies its appeal process; use the carrier appeal route for denied benefits or cancellation.
After internal appeals, an eligible denial may be reviewed by an independent third party; not every insurance disagreement qualifies.
The Arkansas standard external-review rule describes a four-month request period after receipt of the applicable adverse-decision notice; ask which notice and procedure apply.
Documents to prepare (confirm what is required): Denial letter and receipt date; Explanation of benefits; Plan benefits and insurance identification; Internal appeal and decision; Relevant treating-provider records
Exceptions and checks:
Questions to ask: What reason does the denial give?; Is this a health-benefit denial, Marketplace eligibility issue or another insurance dispute?; Have internal appeals ended, or does an urgent exception apply?
Next steps: Follow the denial's appeal instructions.; Ask the Insurance Department to identify the correct external-review or complaint route.; Keep proof of submission.
Limits: The Department must confirm the plan and denial are within the applicable route; this does not decide coverage or calculate a personal deadline. [qr-src-ar-health-appeal; qr-src-ar-external-review]
