Living Choices: assisted-living eligibility and uncovered costs
Qualifications and requirements: Living Choices serves individuals age 65 or older, or age 21 or older who are blind or have a physical disability, subject to financial and other criteria.
Level II assisted-living Medicaid requires an intermediate level of care. The waiver does not cover room and board.
Documents to prepare (confirm what is required): Age and residency information; Care assessment information; Income and resource records requested by DHS; Facility participation and room-and-board charges
Exceptions and checks: Individuals requiring skilled care are not eligible for this assisted-living program. DHS describes income-trust and community-spouse rules that may affect financial screening.
Questions to ask: Is the facility a participating Level II provider?; What care level is needed and how will room and board be paid?
Next steps: Ask DHS about eligibility and assessment.; Confirm facility participation, admission criteria and uncovered costs directly.; Obtain individualized advice before transferring assets or establishing a trust.
Limits: Statewide application guidance does not mean each county has a participating facility or opening. DHS and the facility make separate decisions. Prepare documents privately for verified intake.
